How to Register Your Portal Account and Get Help:

1. I provided my email to my clinic:○ Go to patient.navigatingcare.com

   * Scroll down and click “Need to register” and then select “Start here” to request a new
   invitation

   * Search your email for “Navigating Care” (don’t forget to check your spam!)

   * If you still do not see an email, contact [insert clinic name] [insert number] to confirm
   your email on file

   * After confirming, ask to be sent a new invitation.

2. I don’t have an email to provide my clinic:

   ○ Contact [insert clinic name and number] to obtain temporary credentials (if not
   previously provided)

   ○ Go to patient.navigatingcare.com

   ○ Scroll down and select “Sign in with a username”

   ○ Enter provided login credentials to begin registration

3. I need help and more information:
